As Antalya Dental Clinic Turkey – Expert Smile Transformations, we attach great importance to the security of your personal data. With this awareness, we process and preserve all kinds of personal data belonging to all persons associated with the Clinic, including our patients, in accordance with the Law on Protection of Personal Data No. 6698 ("KVKK").

1. Identity of the Data Controller

In terms of KVKK, the "Data Controller" is Antalya Dental Clinic Turkey – Expert Smile Transformations, located at Antalya / TURKEY.

2. Collection and Processing of Personal Data

Although your personal data may vary depending on the health services provided by our Clinic, they are collected verbally, in writing, or electronically through automatic or non-automatic methods, call center, website, online services, and similar means. Your special quality personal data, especially your health data, and your general quality personal data include:

• Identity Information: Your name, surname, TR Identity Number, passport number or temporary TR Identity Number, place and date of birth, marital status, gender, insurance, and protocol number.
• Contact Information: Your address, phone number, e-mail address.
• Health Information: All kinds of health information obtained during the execution of medical diagnosis, treatment, and care services such as your examination data, prescription information, dental x-rays.
• Financial Information: Your bank account number, IBAN number, credit card information, billing information.

3. Purposes of Processing Personal Data

Your personal data constitutes the basis of the health service we provide. In this context, your data is processed for the following purposes:

• Protection of public health, preventive medicine, medical diagnosis, treatment, and care services.
• Sharing information requested by the Ministry of Health and other public institutions and organizations in accordance with the relevant legislation.
• Planning and managing the internal functioning of our clinic.
• Invoicing for our services.
• Answering your questions regarding our services.

4. Transfer of Personal Data

Your personal data may be transferred to the Ministry of Health, Ministry of Labor and Social Security, Social Security Institution, private insurance companies, our direct/indirect shareholders, subsidiaries, affiliates, group companies, auditors, consultants, work partners, and other third parties we receive services from to fulfill our contractual or legal obligations, within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of the Law No. 6698.

5. Rights of the Personal Data Owner

In accordance with Article 11 of the KVKK, by applying to our clinic, you have the right to:

• Learn whether your personal data is processed or not.
• Request information if your personal data has been processed.
• Learn the purpose of processing personal data and whether they are used appropriately.
• Know the third parties to whom personal data is transferred domestically or abroad.
• Request correction of personal data if it is incomplete or incorrectly processed.
• Request the deletion or destruction of personal data.
